Below are necessary forms required for proper registration with Club Idaho Northwest Volleyball and USAV Evergreen Region.  Please click on each link below, then print, complete, sign, and submit to the board prior to your participation in the 2008 season.

Which forms do I need?

Each participant in TRYOUTS is required to bring with them a tryout form signed (by a parent) and player information sheet.

Each player is required to submit the following forms in order to complete their registration for the season.  Letter of Intent, 2008 Individual Membership Form, and Medical Release Form. 

In addition, players and parents must read, sign and submit a parent/player code of conduct contract each season. 

Chaperones are required to submit a Background Check Form and Chaperone Responsibilities Form each year.

Other Volunteers are to submit a signed   Volunteer Form each year.

All Coaches are required to be IMPACT certified and must submit the following forms each season.  2008 Individual Membership Form, Coaches Code of Ethics, and Background Check Form.
